Does Medicare Cover Home Health Care? What Families Should Know

A plain-language explanation of what Medicare does and doesn't pay for at home — and what to do if it doesn't.

This is one of the most confusing questions families face, largely because two very different services share similar names.

Home health vs. home care — the key difference

Home health is skilled, clinical care — nursing, wound care, medication management, physical therapy — ordered by a physician to treat a condition or support recovery. Home care (personal assistance) is non-medical help with daily living: bathing, dressing, meals, errands, companionship.

What Medicare generally covers

Medicare may cover intermittent skilled home health for eligible patients who are under a physician's plan of care and meet program requirements, when services are provided by a Medicare-certified agency. Coverage focuses on skilled needs, not ongoing custodial help.

What Medicare generally does not cover

Long-term, non-medical personal assistance — help with bathing, meals, and housekeeping on its own — is typically not covered by Original Medicare. Some Medicare Advantage plans offer limited supplemental benefits, so it's worth checking your plan.

If Medicare doesn't cover what you need

  • Long-term care insurance often covers personal assistance.
  • Texas Medicaid programs may cover personal assistance for eligible people.
  • VA benefits such as Aid & Attendance may help veterans and spouses.
  • Private pay lets you choose exactly the hours you need.
